What does YOUR family need?

 

Sawtooth Mountain Clinic, in partnership with Gichi-Onigaming (The Great Carrying Place) Grand Portage Band of Lake Superior Chippewa, Cook County Independent School District 166, and Cook County Public Health and Human Services, recently received an MDH “Local Community Resource Hub” grant.

This funding allows us to create an access point for families and caregivers with children prenatal to five years that will connect families to services and support.

To do this successfully, we need your help to find out:

*What support parents or caregivers have or need,

*Your ideas for building community-based, whole-family approaches so that families have what they need to thrive.

There are two ways to share your ideas with us:

*Virtual Listening Sessions with a bonus- Participants will receive a $25 gift card by mail after the sessions.
