The Sawtooth Mountain Clinic is once again offering a sliding scale payment plan for its patients. Suzanne Jordan, coordinator of the program, said the clinic is in its 31st year of providing the Primary Care Sliding Fee Scale Program. This program is made possible through a grant from the US Dept. of Health and Human Services to Community Health Centers.

The entire process, as well as all records, is completely confidential. Applications are submitted annually and reviewed by the clinic. Income guidelines are revised by the federal government each year.
Anyone with questions regarding the program or eligibility may call Jordan at the clinic, 387-2330, extension 32. The following chart shows the maximum income allowance for eligibility based on family size.
Who may apply:
• Patients at the Sawtooth
Mountain Clinic whose income is within the federal income guidelines, regardless
of current insurance
coverage.
• Singles, couples or families
are eligible to apply.
To apply:
• Pick up an application at the
clinic reception desk or call
to have one mailed to you.
• Complete the form and
attach a photocopy of your federal tax return record, a
W-2 statement or other documents
to verify income.
• Call Suzanne Jordan, coordinator
of the program, for an interview.
