Sawtooth Mountain Clinic offers sliding scale for fees





 

 

For the 36th year, Sawtooth Mountain Clinic will be offering its Primary Care Sliding Fee Scale Program. The program is made possible through a grant to the clinic from the U.S. Department of Health and Human Services.

Who May Apply for Sliding Scale?

. Patients at the Sawtooth Mountain Clinic whose income is within the federal income guidelines, regardless of current insurance coverage.

. Singles, couples or families are eligible to apply.

. Patients may have other insurance primary, sliding fee can help with deductible and copays.

How do I apply?

. Pick up an application at the clinic reception desk or call to have one mailed to you.

. Complete the form and attach a photocopy of your federal tax return record, a W-2 statement or other documents to verify income.

. Call Steve Fleace, coordinator of the program, for an interview.

. The entire process, as well as all records, is completely confidential.

How long am I eligible for the program?

. Applications are submitted annually and reviewed by the clinic.

. Income guidelines are revised by the federal government each year and Sawtooth Mountain Clinic must be granted the funding by the government each year for the program based on its fiscal grant year of June 1- May 31.

. All applicants are terminated on May 1, unless new application is in place.
