Light Up a Life

Care Partners of Cook County invites you to participate in its second annual Light Up a Life event this holiday season. Light up a Life events take place throughout the world in the weeks running up to Christmas, giving people an opportunity to remember the lives of friends and family. Many are organized by local hospices and are open to anyone, whether their loved one was cared for by a hospice or not.

While volunteers and community groups have worked for years to establish a hospice in Cook County, the challenges of funding and licensing a hospice in a small community have led to a simpler approach. Care Partners of Cook County is a collaborative of the North Shore Health Care Foundation, the Cook County North Shore Hospital, Sawtooth Mountain Clinic and other health and community service providers. Its mission is to enhance end-of-life and palliative care and expand services to the frail elderly in Cook County through a community-based approach.

Community members are invited to make small donations to honor loved ones. Each $5 donation will light up one bulb in memory of one person on a Memory Tree. All donations will go to help Care Partners develop its palliative care program. Donation forms are available at stores throughout the county as well as at the library, clinic and hospital. Forms can also be printed from our web site, www.northshorehealthcarefoundation. org/carepartners or see coupon on page A9.

On Thursday, December 2 at 6 p.m. the tree in front of the Sawtooth Mountain Clinic’s main entrance will be lit to honor loved ones. The treelighting ceremony will include readings and music and is open to anyone Refreshments will follow.
