Help with health care directives offered at Honoring Choices

The North Shore Health Care Foundation is presenting a community forum, entitled Honoring Choices, Completing Your Health Care Directive. This free event will be held at the Arrowhead Center for the Arts on Saturday, June 14, from 9:30 a.m. to noon. Refreshments will be served.

It is important to make a Heath Care Directive as it helps you receive the health care you wish and it also makes things easier for your family and physician. This is an important document to make your choices known in the event you become incapacitated.

Upon arrival on June 14, you will be given a copy of the Honoring Choices® Health Care Directive. There will be a panel of professionals who will give insights concerning their area of expertise.

Panel members will be Ruthanne Hedstrom Voss, Attorney at the Mathison Law Office; Dr. Milan Schmidt, physician with Sawtooth Mountain Clinic; Jeannette Lindgren, RN, Care Coordinator, Care Partners of Cook County; Hilja Iverson, Social Worker at Cook County North Shore Hospital & Care Center; and Pastor Father Seamus Walsh, St. John’s Catholic Church.

 

 

A time will be designated for breaking down into small groups so you can pinpoint your area of interest or to gain more insight. Choose one or several groups to take part in.
