Grand Marais Public Library is set to host Completing your health care directive, presented by Marnie Hovland from Care Partners on Wednesday, January 16, beginning at 6 p.m.
Everyone should have a current Health Care Directive, however many struggle with completing the forms. Here’s a chance to demystify the process, get your questions answered and get those forms done!
Participants may wish to read through the forms ahead of time and start thinking about their preferences, especially who you might pick as a Health Care Agent. These forms may be found at the website www.carepartnersofcookcounty.org under the section titled “Upcoming events.”
On the 16th, trained facilitators discuss the important points and answer general questions. Then, stay if you wish, to complete your form, get specific questions answered, have it signed, and make copies. Or, if you cannot complete the task in the amount of time available, you may take your forms home, armed with knowledge of what you need to accomplish to complete your work. Follow-up appointments with Care Partners are also available.
